Why "1mg" has no single unit answer
There is no fixed number of units for 1mg of semaglutide until you know your vial's concentration in milligrams per milliliter (mg/mL). The dose is fixed at 1mg, but the syringe is marked in units of volume, and the volume that holds 1mg shrinks as the vial gets more concentrated. How this is calculated
Two steps convert any milligram dose into U-100 syringe units:
- Volume (mL) = dose (mg) ÷ concentration (mg/mL)
- Units = volume (mL) × 100
Combined into one line: Units = (dose in mg ÷ concentration in mg/mL) × 100. For a fixed 1mg dose this simplifies to 100 ÷ concentration, which is why a 5 mg/mL vial always lands on 20 units and a 10 mg/mL vial on 10.
Volume = 1 mg ÷ 5 mg/mL = 0.20 mL. Units = 0.20 × 100 = 20 units. Draw to the 20-unit mark.
Volume = 1 mg ÷ 2.5 mg/mL = 0.40 mL. Units = 0.40 × 100 = 40 units. Twice the volume of the 5 mg/mL vial.
Because units track volume and not the drug itself, the same 20-unit mark would hold a different milligram amount at a different strength. That is why 10 units does not always mean the same dose.

The pattern is inverse: doubling concentration halves the units. A more concentrated vial means a smaller, harder-to-read volume for the same 1mg — at 8 mg/mL you are reading a half-unit, which is exactly where mistakes creep in.

If you do not already know your concentration, derive it from the milligrams in the vial and the bacteriostatic water you added:

Concentration = 5 mg ÷ 1.0 mL = 5 mg/mL. 1mg dose = 100 ÷ 5 = 20 units.
Concentration = 5 mg ÷ 2.5 mL = 2 mg/mL. 1mg dose = 100 ÷ 2 = 50 units. Same vial, same dose, but 50 units because more water was added.
Concentration = 10 mg ÷ 2.0 mL = 5 mg/mL. 1mg dose = 100 ÷ 5 = 20 units. A 10mg vial at 2 mL behaves identically to a 5mg vial at 1 mL.
